Cap and screw is part number 1158DP0BLK. Bulbs are part number 0071DP0CLR. Lenses is part number 0278DP0WHT. We do not list them but we can look into a special order.

Can the 2 bulbs be controlled independently.?

They are designed to be controlled as you need them to. If you need both on while traveling you can, if you need just one on then you can do that as well.
